Do you think it is possible the OP was not referring to e-mail notification? I find myself posting on fewer and fewer threads because I ask myself if I want the thread to pop up on my profile page for all eternity.
I have requested and others have as well to be able to delist myself from a thread when it becomes tiring. Is the reason such a feature has not been added because it might reduce the web 'hits' on this site? Frankly, I would find myself making more comments if I new I could extricate myself from receiving continual updates once I am tired of the thread and have moved on.

Here's what I wrote:
We do want to build the following feature. It's something many users have been requesting for a while, and is very much on our radar.
- If I participate in a discussion, but I decide later that I no longer want to "follow" it (and get notifications about it), I can un-follow it.
- If there is a discussion that I have not participated in, but I want to follow it (and potentially get notifications about it), I could add it to my "following" list without participating.
Unfortunately, I still don't know what the timeline is for building this, but hopefully soon. The delays are definitely not purposeful.
To answer your very first question, my idea is that your "following list" would be what you see in your profile, and email notifications would be based on this. So it would address the issue you raise of threads popping back up on your profile.
