i travel there for business often and stay at the Omni, and here are couple of places I go. (Keep in mind they aren't hole in the wall since most of them are downtown).
The Republic at top of the Omni is pretty good. I enjoyed the Buffalo Chop.
For casual, I have been enjoying Brewster Street Icehouse, right next to the baseball stadium. Live music there on some nights. Solid burgers, sandwhiches, and wings.
Vietnam right behind the Omni is really good.
I used to go to Katz for steaks, but they are in complete disaray.
I also enjoy having a beer at Dr. Rockets with great live blues playing.
Sometimes would go to Havana Club for a glass of wine.

I see our replies are some 6+ years after the original post so maybe this will just develop into our faves of CC! I have to second the Green Papaya: only been there once but will definitely go back. My family likes the El Playas for seafood and stuffed avocados. We ate at Aladdin's a couple of weeks ago and the gyros and shawarmas were surprisingly good. Maggies has excellent pizza. Wallbangers has a tasty burger (I haven't tried Snapkas yet). I'm sure there are more that I'm not thinking of but all-in-all, there are some decent restaurants in this city that seems to live and die by the buffet.
