Etiquette questions re formal dining in San Sebastian

We are having lunch at Arzak in September and couldn't be more excited. I just have two questions:

(1) What is the dress code for men? Would a polo shirt, khakis, and black dress shoes be appropriate?

(2) What is the appropriate amount to tip for good service at a restaurant like Arzak?

    Arzak is probably the most relax and informal Michelin 3 star restaurant in Spain. Your proposed outfit is fine for a warm September lunch. As for tipping, there is much debate but I would go with the previous poster. I usually leave an extra 20euro for two people.

      - i would wear a nice long sleeve shirt instead of a polo, and it possible a jacket (more maybe a nice polo and jacket at lunch time). (my goto dress code is _nice_ shoes, _nice_ jeans, nice shirt and jacket, no tie)

      - Tip should be included in the menu prices (as they are in most places in Europe), but nothing prevent you to add a little bit more.
