Popup survey keeps coming back

Every time I visit the site recently, even if I'm logged in, I get the same popup survey.

I've filled it in several times, and I'm not blocking cookies.

At first, it would go away once I filled it in, but today, even after I fill it in, it returns again and again, every few minutes.

In addition, the report problem link (marisa.rosen...) bounces email.

Quick response from the internal team about this! My contact says, "It looks like the cookies are not working properly. We will investigate and take this down until we get it figured out."
