The food is mediocre and the prices are relatively high (considering that the food is in fact....mediocre).
People like the Saudi Ambassador go for the ambiance. Cafe Milano is one of those places where you can go and be seen---if you choose. Or you can have a private conversation with a recognizable DC figure and your privacy will be protected and taken care of.
There are a number of much better Italian restaurants in DC (Bibianna in particular is a personal favorite)....places where you will get a wonderful meal if in fact an assassination is attempted is this is your last supper. :)
