ISO : Curds for Mozarella
I hunted for this a long time too and ended up finding this place through a friend of a friend:
Not in curd form but a big block of raw curds can be bought from them, just call ahead. I tried doing it once and the cheese was fantastic fresh, but I could really tell that I over worked it when I had it the next day, too much fat ran out and it was sort of tough. But you can give it a go yourself. Good luck.
They also sell the scamorza curds raw that is used in some sort of ethnic dessert, but I don't remember the details.
Oh and they also sell the buffalo moazzarella that I believe they source for Terroni's and Pizza Liberato. It's worth the drive just going there for that.
