Alvin Garden for Hunan and S&W Pepperhouse for Sichuan.

+1 on Alvin Garden -- spiciest food I've ingested in Canada, tasty and inexpensive too. Still haven't had my head melted by any of the Sichuan I've tried in recent quest to understand the real thing but I'm sure they have the technology to do so at S&W :-).

My brother's girlfriend is from Thailand and often eats at the Noodle Box. I believe she asks for her spice level "Suicide Hot".

does your brother girlfriend know of anything else that is just as spicy?

The problem with Noodle Box is that each spice level just means more sambal oelek. This means that if you order the suicide hot, your just getting food that tastes like sambal (I think suicide hot is 3 tablespoons).

Ask for Indonesian hot-your life will never be the same.

Of course the same applies to most Thai and Indian places-if you want exceptionally strong chile they'll do it for you but you have to ask.
