How do I list my restaurant on Chow.com?

I have searched and cannont find out how to get it listed. Please help!

These fields are yes/no only:
Delivery, Outdoor seating, Reservations, Late Night, 24 hours, Private Parties, Counter-service only, on-site parking, serves lunch

Since I am having issues adding new entries to the database, I may send you new places by email...do I have to follow the format above exactly? And do I need to fill out all the fields? (ie Can I leave some blank)?

The minimum I need is a name and full street address. Everything else is optional. You don't have to follow that format.
