Fine Fare on the LES carries it, but don't make a special trip or anything. I'm pretty sure any regular (i.e. not Whole Foods or Fairway) grocery store would have cavatappi/cellentani, and Barilla and Ronzoni even have "pasta finders" on their websites.

Happened to be in Harlem Fairway today and remembered this post. Without looking hard (or at the artisanal brands), I saw Ronzoni cavatappi, De Cecco cavatappi, and Barilla "celentani" (which is their name for the same shape).
