Everything is 20 - 25 minutes when it's in the planning stages. When the rubber hits the road, you get stuck in traffic, the train is late, there's a problem on the tracks, the cab driver is stuck on the phone with his divorce attorney, you name it.
As a Philly boy born and bred, I would not leave the airport for a 3.5 hour window. You could get some tasty treats - Roast Beef at Nicks at 20th and Jackson, Cheese Steaks at any number of special places, etc. But to me, it's not enough time for a stress free lunch. Suggest you leave it for when you're visiting Philly. If you have no intent ever to come back, and have heart set on this, I would do the train recommendation for the reading terminal market. That gives you variety and local flavor, without risking a poor recommendation.

I have to agree, it's definitely cutting it close. Definitely not enough time for a sit-down lunch in Center City. If you really want to give it a go, I suggest getting a cab to John's Roast Pork (roast pork or cheesesteak), Tony Luke's (cheesesteak or chicken cutlet) or even Texas Weiners, get something to go, and hop right back in a cab. All three are fairly close to 95 South (Tony Luke's the closest out of the 3) so it's easy access.
If you decide to stay at the airport, check out the attached Marriott. They may offer lunch. I had a Mother's Day brunch there and it was pretty good.

3.5 hours is plenty of time to get to Center City and back by either cab or train assuming you aren't taking an international flight! You need to be back an hour before your flight to get through security. I agree with the Good Dog or Reading Terminal suggestions, but I'll also add John's Roast Pork - on Snyder Ave near Front Street. Easy off and on I-95 nearby and arguably (not that I'm starting one) the best Roast Pork sandwich and Cheese steak in Phila. It closes at 2:30, picnic table seating outside which works with tomorrow's weather forecast!

3.5 hours is cutting it a bit close, time-wise, to leave the airport, get to the city, eat lunch, get back to the airport, and most importantly, get back through the security screening in time. Also cab fare would be at least $20 each way, that would be a pretty expensive lunch. So if I were you I'd eat at PHL, but I don't fly that much, so I may be out of touch regarding how long it takes to get through security.
If you think you can make the time frame, I'd recommend taking the R1 airport train to the Reading Terminal Market, rather than cabbing it. It's 25 minutes each way, much cheaper than a cab and there's something for everyone at the RTM. I'd get a DiNics roast pork sandwich for lunch and maybe a pastry from Termini's for the second leg of your trip. I'm not sure when your flight is but the R1 runs a train every 30 minutes between the airport and the city (and vice versa) 7 days a week:
