Thanks for all the details on Langer's, everyone.
To me, the main question one gets asked in a truly Great pastrami establishment is

"Y'wannit LEAN or y'wannit GOOD?"

Is it possible to request the level of fattiness in a pastrami order there?

You can ask for it extra lean or extra fatty. You have to pay extra for extra-lean because they just trim off the fat and they have to use more of it to make a sandwich.

Make sure to specify you want it hand-cut.

And at Katz's the question is often "You want it good, or good for you?" If you order it extra lean you get a smaller sandwich out of scorn. :P

Yes, it's possible. Two years ago, my family was in town and we all went out to lunch at Langer's. My sister ordered it lean, and couldn't understand why everyone else at the table was in ecstatics (until she took a bit of mine)
