I really enjoyed the plate lunch at Itsu's Fishing Supplies and the food at Ken's House of Pancakes. Hilo Lunch Stop looked good too and if I had more time I would have tried it.
Kuhio Grill has tasty Hawaiian food...the laulau has beef which I thought was a bit odd, but it was very tasty.

No one is going to mention Cafe 100? That's about as local as it gets. Loco moco the big thing but I like the combo plates. There use to be the lau lau place, Kaupena Ono Food but I think they closed a while ago.
