Dinner recommendation for SF?

Sorry to post such a broad query, but I am having dinner next week with a friend I haven't seen in a long time. I want to go someplace in San Francisco that is really good - very special. It's OK if it is expensive.

I've been out of the loop and don't know what the good restaurants are in SF anymore, hence the post, even though I live here. I just haven't kept up as places have opened, closed, and changed chefs.

Most important is that the restaurant be quiet enough for normal conversation.

It should be East of Twin Peaks.

I don't like anything too spicy or exotic (I'm from the Midwest), no Thai or Vietnamese food, please.

Great desserts are a huge plus.

Yes, I know this is vague and annoying (another "Where should I go to dinner?" post), but I don't know where else to turn.

My friend is a teacher, and I would like to celebrate the end of the school year by taking her to a four star restaurant, something really special.

          Second on Acquerello. Italian with French influence. Great food, stellar wine list, impeccable but warm and friendly service, very special atmosphere. Not in a hotel.
