I second Craftsteak and Balthazar. The former isn't the traditional interpretation but the beef quality is great. Balthazar is authentic and very good as well.
Blue Ribbon Bakery and Les Halles (mentioned) also serve decent steak tartare.

I had good steak tartare at Bar Boulud, the bar room at the Modern in the MOMA and Pere Pinard.
