Columbian Sauce with red pepper flakes and vinegar?

We had a delicious sauce/salsa at a Columbian restaurant and I'm trying to make it at home. It was already on the table when we sat down, so I think it's served with every meal. It was very spicy with some green ingredient, red pepper flakes and vinegar.

Does anyone know the name of this sauce or how to make it? It's addicting!
