Tilapia Salad at De Mole.
Not make your own. But it's super good.
I wrote a post just about this salad: http://chowhound.chow.com/topics/527689

It's not make your own, but Aubergine Cafe has great salads. There are cheap salads with decent ingredients at The Met which are on ice near the produce. . . Quaint has tasty options but for dining in only.

re: cobbler
New York City Eats doesn't get a lot of respect on this board, but its salads are really fresh and good. I ordered a Sicilian last week - baby spinach, fresh mozzarella, roasted red peppers, asparagus, avocado, and capers. Delicious. My only complaint was with the peppers, which were jarred, but otherwise a great salad.

I'll second the NY Eats rec. When my kitchen was being redone, I pretty much lived on those salads, and they give you enough for two meals, easily. The negative rep probably comes from main dishes, burgers, especially. There have been times when they've been great, but they go through phases when they change line cooks, and then bad things happen to the burgers.

It's not really what she's looking for, but De Mole has lots of good salads. Better than most of the DIY salad places I've been to.

re: catherine
agreed. I love the Demole salad with shrimp. The one with chicken can vary from juicy and moist to dried out. But the shrimp is always consistently good, and the Tilapia gets raves as well. I've also had them make customized salads for me when dieting which has been really nice of them.
