Has Sushi Zo ripped you off?

I want to start out by saying that Sushi Zo consistently offers me the best sushi I've ever tasted. I love love love it.
But I've been twice now, and each time I order and am billed for food that never arrives! The first time they ran out of toro, but the sushi still appeared on the bill. The waitress quickly removed the toro from the bill after I pointed out the mistake.
During my second visit, when the bill arrived, I didn't remember having eaten the 2 pieces of butterfish I ordered and was charged for. I asked, and the chef said that he did prepare the butterfish, so I just paid for it. On the way home, though, I realized that I definitely did not eat any octipus, which I also ordered and paid for. This made me question the the integrity of the chef's assurances regarding the butterfish.
Has anyone else had similar experiences? If so, I'm tempted to bring a notepad with me during my next visit so I can keep track of what I eat!
i thoroughly check the bill about every other time i go.
i have NEVER been charged for something that wasn't delivered.
there have been mistakes on my bill. i've only been there 3 times, and it was on my 2nd that extra items were on the bill that i didn't consume. i brought it to the attention of the waitress and they were taken off the tab. i should've checked the bill on the 3rd visit but forgot? i recall reading some other posts here on ch that billing error has happened to them too.
Boy, all of these responses of multiple mistakes are troubling. I am going to have to scrutinize the bill more closely from now on, though I have never had a problem in the past.
