Hmm, just got back from there last month. The food near Manuel Antonio definitely doesn't represent the rest of Costa Rica. Everything around there is very touristy (expensive) and not very authentic. We stayed at the Si Como No resort and ate at the hotel restaurant pretty much every day (food was good) and also for a little fun (and to see a piece of history) you may want to check out El Avion Bar and restaurant. http://www.costaverde.com/avion01.htm

Hope this helps. Will you only be visiting Quepos?

We stayed at the Mariposa which is worth going to for the view, breakfast is very good. Food at Si Como No was very good too, I remember. There's a lovely little coffee shop a little way down the road, can't remember the name though (coffee is great in Costa Rica!)

I know a place that I wouldn't necessarily call "good" (although I didn't think any meals in Costa Rica were really GOOD as much as OK, except for some of the food on the Caribbean side), but it's dirt cheap and thoroughly local. Costa de Oro was recommended by my hotel manager, and then the one time I went, I ran into him and his friends! It's about 1000 colones for a casado so big I couldn't finish the plate, but they have an actual menu with more options too.

Are family was in Manuel Antonio August 2009 Our hotel recommended Marlintinis. We had a great meal. The seafood was really good I had red snapper and my wife had the Seabass incrusted in Macadamia nuts very tasty. Kids had chicken and ziti. The staff was very friendly and the open air gave the place a very Costa Rica feel. Would definitely go back there again

The best meal I had on my last trip to Manuel Antonio (and actually the best meal I've ever had in Costa Rica) was at Kapi Kapi. The Mangos Foster and the Macadamia Nut Crusted Mahi Mahi with Sweet Chili Plum Sauce (in particular) were to die for...and the restaurant is beautiful.

I also really enjoyed The Marlin Restaurant down at Manuel Antonio beach. The coffeewood smoked trout with creamy dill sauce and crostini was so good I had to go back for more. The Marlin is a very laid back beach bar environment across the street from the beach with a kind of Costa-Rica-Meets-Key-West environment. Fun. Good sangria, too.

I wasn't as impressed with my meal at Marlintini as the previous poster was (our seafood was overcooked), but maybe it was an off night.
